Issue was a bad O2 sensor. fuel was dumping into the cylinders on the right bank. A new sensor fixed the issue.

I believe you can also use the newer GT500 coils.

They're actually surprisingly cheap when you buy an 8 pack. Just go over the listings with a fine toothed comb and make sure they're not Chinese...

My son owns a truck parts business and he has a very expensive 2000.00 scanner. That could not detect what cylinder.
