Is the rattling of the timing chains periodically anything to worry about

Depends.
If it's just due to tensioner leak-down and only occurs on start-up after sitting, then no. If you can hear them at all when the engine is running, then yes.
Yes if the guides have worn through. When in doubt, replace them

Better to replace the chains and guides than loose the motor .
