Nordstrom automotive on E-Bay. Got mine for 46.95 in Dec. 48 hr. shipping! So far so good. easy install also!

Yes to all questions. the mirror comes with a black "scull kap"

sorry for the delay in responding. The cap was not useable so I did buy a new mirror from ebay and will be installing it today
