Not enough info.

How many miles? When was the last time the fluid was changed? Does it happen from a rolling start, or just from a dead stop or both?

Doesn't sound normal.

When was the tranny fluid changed last?

Check the fluid level.

Does the fluid smell burnt?

Drop the pan. Is there alot of stuff in the pan?

Is there a custom tune loaded? Maybe torque converter lockup occuring. Oh yeah, NEVER put the pedal to the floor in 4th gear. Always have OD off when playing around.

Is there a max speed that you can safely lock out the O/D ?

I dunno, I've clicked it off when going about 65, to make a quick pass on a two lane highway. Always heard it's not good to floor it with OD on. I always drive around town with the OD off, mostly just use OD on the highway when cruising.

When you are at 50MPH and up (I think) you want to take the O/D off before gluing the pedal to the floor. If you don't the tranny will shift from 4th to 2nd, which is not good for the tranny.

I guess my question is, can you safely lock OD out if you're already in OD ? Like hitting the OD lockout button at 65 - ok or bad idea ?

I've wondered the same thing 98% of the time you're cruising along with OD on, but then some wanker jumps in front and you need to mash it.

Ya know. Because.

Click the OD off first before the mash or no?

Think of it this way. Your OD is a band much like a 70's era brake shoe. Enough said. Don't do anything with it except cruise. As for taking off in OD, that doesn't do squat because you aren't taking off in 4th gear, er band... Your fine all the way up to the end of 3rd to unleash hell. I have made the mistake of nailing it in OD plenty of times without fail, but always fearing.. My last track outing was with a 4.30 gear that required OD to finish the 1/4. Three passes at WOT, still nothing. Lucky. And finally yes, you can click it off any ole time you want assuming your not gonna go completely past redline into third. IE well into the triples with a factory gear.
