See if the rivet is broken off from the actuator.

Small flathead screwdriver to pop off the handle surround, upper grab handle ends, and master switch plate. The rest uses a Phillips head.

I've seen it happen a few times. I guess after a while, the rivet gets weak and breaks off.

Could be the rivet as Roy said or the actuator came out of the bracket or the lock for the actuator rod is broken.

It a 1/4" rivet which uses a BFRG. I used a 1/4 / 20 stainless steel bolt, washer and insert nut when I have to replace them.

Yup you found the issue.

I just got a screw, bolt, washer and I was done. $1 fix!
